Please complete the steps below to submit a referral for an assessment

Step 1

2. Parent/Carer Informed Consent for Involvement

Step 2

Please upload your completed documents to the secure form linked here and submit

Step 3

Edvocation will process these files and contact you via email, with initial digital assessment links for you to complete

Step 4

Once step 1-3 are complete, a calendar link will be emailed to you for you to book an assessment visit.

This is a face-to-face visit and opportunity for our assessor to undertake observations and work on some individual activities with the child in their school setting.
